Look At My Hair module crash, bug reports page not found

I tried installing LAMH and there were *TWO* fields for an installation code and an activation code?? Not sure...

Anyway I ran the module and DAZ Studio 4.5x wasted no time crashing. When I tried to submit the saved crash report ZIP file to the supplied URL, I got "page not found".

Should I be concerned about this, I wonder... thanks, Roman
