Why does this keep happening? (v4 pose issues)

I am using V4 and using V4 poses but it gets the arms and chest area looking weird, particularly with this character.
He has Vittorio injected but I don't see why that would make such a difference?
In pic 1 (pic of him ontop of another vittorio V4) you can see his shoulder is messed up.
In pic 2 (in bath)
you can see near his chest around his underarm is pointy and his biceps look a little messed up.

I tried using the morph thing in Poser (which I am using, not Daz) and smoothing it but it does nothing.

    I don't know what causes it, but I have seen it on V4 without Vittorio injected, so that's not it. I think it happens when I apply a new pose to a character that had previously been saved.

    I'm not sure but I think this is caused when a Pose exceeds the limits of one of the bones. The mesh still follows the Bone even if the limit is passed causing the mesh to deform because it is linked to the bone.

    That would only apply to the top pose. Not sure what happened in the last one.
