How do I remove custom action items?

I have an item that I put created a custom action for a long time ago but I'd like to get rid of it and don't know how.  Is there a way to delete these items once you put them there?

Window menu > workspace > customize

On the right side go to menus tab, then expand main menu, then scripts. Right click what you want to remove and click remove item. Then hit apply and accept.
