2d image to 3d

When attempting to create an icon to 3d, my image comes up with the image being indented rather than sitting on top.
Any suggestions?
Also, can't seem to find image saved in Bryce 7. I searched folders in order to 'open', but no idea. Again suggestions will be greatfully appreciated.
Found it, but the icon question is still active.

    Think of middle grey (rgb 128, 128, 128) as flat ground, darker shades of grey as below ground and lighter shades of grey as above ground.
