What causes dynamic clothing to start... melting(?) off the body?

That's the best way to describe it. It happens mostly with the bikini bottom and "heavy" dresses. They drape nicely... and then just keep stretching like they're made of pizza dough. I don't have any screenshots at the moment, sorry :(

    If you mean what I think you mean this happens when the clothe gets trapped between two body parts, such as the upper arms and the side of the chest or the thighs, and can't get away - particularly in the case of the arms the mesh may actually be interpenetrating - so the simulation pushes it out through the mesh and drags another part of the cloth inside, which then gets pushed out and so on. If that is the issue your best bet is to adjust the pose or to use a DForm to keep the hidden surfaces of the wearer apart so the cloth has room to move.
