Geometry "separates" from trim when smoothing modifier is applied

Some geometries "break apart" when a smoothing modifer is applied. Is this something I can fix using a geometry editor? I have Hexagon, but I've not explored it yet.
The attached example shows the fabric pulling away from the trim. If I lower the smoothing iterations, it is less noticable.

The trim is most likely not welded to the rest of the outfit. I don't think you can change that with geometry editor.
