For breathing, I'm not sure if there are any that would work well, but you could check your character for morphs that slightly enlarge/raise the chest?
For the genitals, I think possibly you want to research geografting, although I'm not sure. You can certainly create them in Hexagon, it's probably a major project if you have never used Hexagon though. You'd have to texture them too. No idea how to fit them to the character though, somebody else will have to answer that. For the avatar, click on the gear icon in the upper right corner of the forums, select your username from the menu, then click the second gear (that's within the highlighted bar that includes your username on this new page), then select Edit Profile from the menu, then select "Change my picture".
