I have uninstalled and reinstalled everything with DIM, but I still get
Plugin failed to load!

Which version of DS are you using?
Decimator plugin would only work with the latest DS version, i.e. General Release 4.22.0.16, Public Build 4.22.1.150. You either upgrade your DS General Release, or install the latest Public Build, or send a ticket to Daz asking for the older version of Decimator which is applicable for the DS version you're using.
